Since day one, ESO EHR was designed to help agencies quickly and efficiently capture patient records that meet NEMSIS data standards. In doing so, consistent data entry was key. This consistency removed any friction from the process of staying compliant, but limited agencies’ ability to capture data necessary for obtaining grants, research, or any additional medical director requirements.
The ESO community vocalized a desire for more native customization, so the ESO Innovation Team got to work designing a tool that aligned with agencies’ needs, allowing this much needed flexibility without compromising the ability to meet NEMSIS data standards.
The end result was Agency Forms. A team effort between ESO and the ESO community, Agency Forms delivers the customization that agencies require while upholding the continued quality of data.
Your team can now collect custom data fields with Single Line Text, Multi-line Text, Multi-select, and Date/Time – well beyond your standard ePCR. This allows you to track grant compliance and document local operational reporting, clinical research, quality improvement, and community health initiatives without any extra steps and without having to wait for fields to be requested enough to be added to the product roadmap.
With Custom Rules, you can create validation rules tailored to your documentation requirements. These rules use the conditions you define to ensure that required fields, flowchart entries, or other documentation is completed within your Agency Forms when your specified criteria are met.
How Agency Forms works
Your agency shares your desired customization with your ESO team, who in turn build and validate your rules and forms on your behalf – ensuring you get exactly what you’re looking for without compromising your data integrity. You review the draft and approve, then both the rules and forms are published. The data integrates directly with ESO Insights and Snowflake, so you can report on it right away.
For your field teams, custom forms appear within their normal ePCR – no new apps or extra training needed. Rules Builder allows you to require customized forms based on incident type, impression, or other triggers, making it easy to continue to collect consistent, accurate data in the field.

Agency Forms is a new offering for ESO EHR customers that comprises custom forms and custom rules:

Yes, Agency Forms, including custom forms and rules, are available as a combined purchase and includes the assignment of a Client Success Manager at ESO that will support you using the rules and forms builder to build and publish the needed forms and rules to your ESO Admin tenant.

How do Agency Forms appear to field providers?
Agency Forms appear directly within the normal EHR workflow. Field personnel complete them as part of chart documentation without needing a separate application or workflow.

Can a custom rule require the completion of a custom form?
Yes, a custom rule can make a custom form conditionally required based on criteria such as incident type, impression, or other configurable triggers.

Not currently. Form and rule creation is performed by ESO Professional Services. Future roadmap plans include broader customer administrative access.

Agency Forms, including custom forms and rules, are currently built by ESO Professional Services to ensure rule quality, reporting integrity, and workflow consistency.
Why does ESO build rules for customers today?
Rules can impact documentation workflows, reporting accuracy, compliance, and downstream behavior. ESO currently reviews and configures rules to help avoid unintended consequences.
How does a customer request a custom rule or form?
The customer works with their ESO Customer Success Manager, Account Executive, or Professional Services team to define the requirements. ESO then evaluates, designs, tests, and deploys the rule or form.
